We are offering seasonal flu vaccinations to all eligible patients at the practice.

By proceeding to book and attend a flu vaccination appointment, we will assume that you:

• Have read and understood the relevant information about the flu vaccine
• Have had the opportunity to ask any questions you may have
• Consent to receiving the flu vaccination
